What We Need To Know FIrst
• Name of decedent (deceased person) • Location of the decedent (street address)
• Your name and phone number • Name and contact numbers for next-of-kin
• Physician’s name and phone number
As Soon As Possible
Personal information about your loved one is required for the California Death Certificate. Our funeral home staff will assist you in preparing and ordering the right number of copies.
To begin the process you can either fill out the online form or download a PDF of the Vital Statistics, fill it out, print it and bring it with you to our funeral home when you visit for the arrangement appointment. (If you can't print it, we'll help you fill it out at the appointment.)

Clothing for the deceased (include undergarments)
Jewelry, eyeglasses, dentures, prosthesis
Veteran discharge papers or DD-214
Pre-paid funeral/cemetery plans, if applicable
Obituary: photo, biographical information. We'll help you write it.
Insurance policies (documents + phone numbers)
